Request To Block Adverse Items of Information as a Result of Human Trafficking
If you are the victim of human trafficking or sex trafficking, you can submit a request for Foley to remove or block adverse information that might appear on your background check report. You can contact Foley at either of the following addresses:

For Foley to process your request, you will need to provide the following information:

Proof of Identity. Foley will need a minimum of two identifiers which may include a full name and either:

Full Address

Social Security Number

Date of Birth

Copy of Driver’s License

Government Issued Identification Document

Trafficking Documentation. You must include at least one of the following:

A determination of victim status from a federal, state or tribal government entity

A court document showing victim status

A determination of victim status from a non-governmental organization or member of a human trafficking task force authorized by the government to make such a determination or
-
A self-attestation that you are a victim of human trafficking. The statement must be signed by a government entity, a non-governmental organization or a human trafficking task force, or a court

- Identify Adverse Information
Victims must self-identify what information on their consumer report was a result of trafficking.
- Preferred Method of Contact
Foley will need the preferred method of contact to communicate either electronically or in writing. Please provide a mailing address or email address.
A Foley representative will notify you once we have received your submission if additional information is necessary to complete the submission or if any of the required documentation is missing.
Once a final determination is made, you will be notified in writing.
